About Home Hospice Care

Home hospice care gives patients the opportunity to receive skilled, end-of-life support in the place they know best. Rather than focusing only on medical needs, our care honors the whole person by meeting physical, emotional, and spiritual needs with respect and understanding.

At Crossbridge Hospice, every plan of care is created in partnership with patients, their physicians, and their families. Our interdisciplinary team provides guidance, symptom management, and compassionate support through each stage of care. With Crossbridge Hospice, home is where care, connection, and dignity come together.

What Home Hospice Care Includes

Hospice care provides personalized, clinician-led support for patients who wish to remain at home during the final stage of life. It combines expert medical care with emotional and spiritual guidance, helping patients find dignity and calm in familiar surroundings.

At Crossbridge Hospice, our interdisciplinary team of nurses, social workers, chaplains, and aides works together to meet each patient’s unique needs. We focus on relieving symptoms, improving quality of life, and ensuring families feel informed, involved, and supported throughout the process.

Clinical Care and Symptom Management

Our nurses and hospice aides deliver skilled medical attention to ease pain and manage symptoms effectively. We coordinate medications, monitor comfort levels, and adapt care plans as needs change, always prioritizing each patient’s quality of life.

Emotional and Spiritual Support

Social workers, chaplains, and counselors provide meaningful guidance and a listening ear. Whether through conversation, prayer, or reflection, we help patients and loved ones find strength, connection, and a sense of peace during this time.

Family Education and Ongoing Guidance

Families receive clear, compassionate communication from our team at every step. We teach loved ones how to provide daily support, recognize changes in condition, and access care when needed — with a nurse available 24/7 for reassurance and help.

Service Areas

Crossbridge Hospice provides home hospice care across select regions, ensuring families have access to experienced, local care teams. Wherever we serve, our goal is to bring skilled support and understanding directly to the homes of those who need it most.

Home hospice care includes a full range of services designed to provide comfort and support at home. This may include visits from nurses, hospice aides, social workers, and chaplains, along with medications, medical equipment, and supplies related to symptom management. Families also receive education, emotional support, and 24/7 access to a nurse for questions or urgent needs.

Hospice care includes regular visits and 24/7 on-call clinical support, but it does not provide a continuous live-in caregiver. Most care is delivered through scheduled visits from nurses, aides, and other team members. If a patient’s symptoms become difficult to manage, short-term continuous home care or inpatient hospice services may be provided for additional support.

Hospice care at home is typically covered by Medicare, Medicaid, and most private insurance plans. These benefits generally include visits from the hospice team, medications for comfort and symptom relief, and essential medical equipment. Our team can help you understand your coverage and answer any questions about costs before care begins.
